In this article, the first case of a TBE virus induced meningoencephaloradiculitis in Belgium is reported. A 23-year old male was bitten by a tick during a stay in Slovenia and back in Belgium he developed severe TBE. While PCR for TBE was negative in both serum and CSF, TBE IgM and IgG antibodies were found in serum and IgG antibodies also in CSF. In addition TBE diagnosis was confirmed by neutralizing antibodies in serum and CSF. The clinical picture of this case and its development over a year has been described in detail. Reports like this one may increase medical awareness for TBE in non-endemic countries, leading to an optimal medical support of the patient and a reduction of the complication rates that accompanies severe TBE especially when belatedly analyzed.
